I understand that participation in Village Forge activities — including but not limited to gaming, physical activities, community events, workshops, and travel — involves inherent risks. I voluntarily assume all risks associated with participation, known and unknown, including the risk of physical injury, property damage, or other harm.

To the fullest extent permitted by California law, I, on behalf of myself and/or my minor child, hereby release, waive, and discharge The Tech Village LLC, its staff, volunteers, and partner organizations (including City Church) from any and all claims, liabilities, damages, or causes of action arising from participation in Village Forge activities. This release includes claims arising from negligence, except where prohibited by law. I acknowledge that California law may limit the enforceability of this release with respect to minors.

In the event of a medical emergency in which I cannot be reached, I authorize TTV staff to seek emergency medical treatment for the participant named in this form. I agree to be responsible for any medical expenses incurred.

Village Forge is a free, open space. We expect everyone — youth, staff, and guests — to show up with respect for each other and the community we're building together. That's it. We handle the rest.

We document the village — photos, video, and stories — to share what we're building with the world. Your participation may be captured and used for TTV outreach, social media, and storytelling. We will never share content that puts anyone in a negative light.

By signing below, the parent, guardian, or participant (18–19) confirms all information provided is accurate and gives permission to participate in Village Forge activities hosted at City Church, Antelope Valley.
